- e-Form customizable fields are placed on any webpage and are completed by the relevant ‘customer’.
- e-Form includes the ability to upload any-size attachments that may be relevant to the form, such as a copy of a birth certificate, or driver’s license.
- Both transport and storage of data submitted is encrypted and secure.
- Does not require an existing website to use SSL to safeguard the information submitted.
- Each form submission triggers an encrypted email message that is sent irectly to the internal designated recipient(s) existing inbox.
- The internal recipient can securely reply to the form submission and engage in a private conversation with the website visitor or client – without divulging their email address.
- The submitted data is automatically entered into any third party application or database.
- e-Form examples include: secured ‘Contact Us’ form, Benefits Enrollment, Loans Application, Student Enrolment, Medical History, Confidential Quote Requests.

The Challenge with Printed Student Forms
- Numerous student forms are sent home each year by each child to be filled out manually by parents (if they get home at all). The forms request the same information as the previous years’ forms and often have to be completed for each teacher or classroom.
- This year the form is once again filled out manually using a pen. The form fields are often unclear and provide limited space. Once completed, the handwrtten form is often illegible and missing information.
- The completed forms – which now include ultra-sensitive and private information – are delivered back to school by the child (talk about secure).
- The completed printed form is manually collected, data-entered, filed, archived and eventually destroyed by the school.
With e-Forms
- Secure e-Form is placed on the school website that can be completed securely online and accepts any-size uploaded attachments, such as immunization records, birth certificate, or proof of address.
- The school sends a secure email to parents directing them to complete the online form.
- Challenge Response Authorization is used to authorize and confirm the parent’s identity.
- The form is completed securely online and any necessary document attachments are uploaded.
- The form provides help, clarification and information for each field. It also ensures that all fields are completed.
- An encrypted email notification is sent to a designated school administrator’s inbox and may include the form submission details.
- The school administrtor can begin an encrypted email dialogue with the parent if additional information, documents or clarification is required. The school staff member can converse with the parent without divulging their email address during the exchange.
- The form submission is automatically and securely uploaded into the school’s existing database.
- If the parent does not complete the form by a certain date, the parent is automatically sent a secure reminder by email.
- The following year, using Secure e-Statements, the school sends the parent an encrypted email that includes an attachment statement summarizing and confirming the child’s information on file. If the information requires updating, parents are directed to the online secure e-form to make changes.
